Since its inception in 1982, the Foundation has been involved in advancing the heart health
communities through education. In 1994, the Foundation further expanded its efforts in this
area with the introduction of the HeartMatters program, a statewide community-based
prevention initiative established as a vehicle to provide information, support, and
encouragement to individuals and families in their pursuit of cardiovascular health and
wellness. Our education programs are typically facilitated by Minneapolis Heart Institute
and Minneapolis Heart Institute Foundation physicians and staff, contracted health
professionals, student interns and representatives from various partner agencies. The
HeartMatters program includes several components including annual special events, a slate
of portable prevention-focused programs and services and a number of strategic community
partnerships and public health initiatives aimed at those audiences recognized as being at
greatest risk for disease development.

HeartMatters Community Programs and Services
The Foundation has developed an extensive menu of portable programs and services for delivery to sites throughout the state and region such as faith communities, schools, healthcare facilities, worksites and community centers. These programs include computer-based risk assessments, health screening services, a community speaker’s bureau, interactive educational displays, individual risk factor counseling services and other prevention-focused seminars and workshops. We continually seek opportunities to share our message of heart disease prevention with new audiences statewide, especially high risk and medically underserved populations, through formal community partnerships and other collaborative initiatives.
